Description

Document an individual employee’s work related medical history.

These records are not personnel records and must be kept in a separate location from employee personnel records as required by the Americans with Disabilities Act.

These records found in all media (paper, digital, or other) may include but are not limited to:

  • documentation of work-related injuries or illnesses;
  • medical examination records (fitness for duty, pre-employment, pre-assignment, periodic, or episodic);
  • documentation of significant health or disability limitations related to job assignments;
  • hearing test records;
  • hazard exposure records;
  • first-aid incident records;
  • vaccination attestations, proof of vaccination records or exemption forms;
  • physician statements;
  • release consent forms;
  • related documentation and correspondence, including e-mail.
Retention and Disposal Instructions

(a) hazard exposure records: 30 years after employee separation, purge; 

(b) audiometric test records: until employee separation, purge; 

(c) work-related injuries and illnesses: 6 years after closed, purge; 

(d) vaccination attestations, proof of vaccination records or exemption forms: 3 years after submitted, purge; 

(e) all other employee medical records: 3 years after employee separation, purge.
