Description

Document the administration of health service psychology internships and supervised clinical experience offered as part of a course of study for the purpose of professional training. These records found in all media (paper, digital, or other) may include but are not limited to: 

  • applications and supporting material; 
  • agreements and/or contracts; 
  • postings/notices; 
  • student resumes or curriculum vitae; 
  • academic transcripts; 
  • audio or video of counseling sessions and supervision meetings captured for the purposes of supervision and training only;
  • interns’ training experiences, evaluations, and certificates of internship completion;
  • notes; and,
  • related documentation and correspondence, including e-mail. 

This series includes some permanent records.

Note: For APA accredited programs, “the program should inform interns of its records retention policies.” (American Psychological Association (2019). Standards of Accreditation for Health Service Psychology and Accreditation Operating Procedures). 

Retention and Disposal Instructions

(a.) Retain interns’ training experiences, evaluations, and certificates of internship completion for evidence of the interns’ progress through the program in perpetuity; 

(b.) Retain audio or video of counseling sessions and supervision meetings captured for the purposes of supervision and training only no longer than one academic term, purge; 

(c.) Retain information and records of all formal complaints and grievances submitted or filed against the program and/or against individuals associated with the program a minimum of one accreditation site visit; reference any related UO policies related to complaints and grievances recordkeeping prior to purge;  

(d.) All other records, 7 years after client's last contact, purge. 

Exception: In the event of a subpoena, audit, legal hold, public records or similar state or federal information request, halt until further instructed any scheduled disposal activities, including purging or transferring material to University Archives.
