These records found in all media (paper, digital, or other) may include but are not limited to:

• federal and state applications and certificates including the State of Oregon Health Division Radiation Material License forms and amendments;
• validation certificates showing fees paid for licenses;
• related documentation and correspondence, including e-mail.
