Description

Document operations of university childcare centers. 

 These records found in all media (paper, digital, or other) may include but are not limited to:

  • complete and current information on each child as specified in OAR 414-300-0040(4) and (6);
  • daily attendance as specified in OAR 414-300-0040(1)(b);
  • personnel record for each staff as specified in OAR 414-300-0040(1)(c) and consistent with retention requirements specifed in UO Retention Schedule for Employment Records;
  • documentation of the death of or injury to a child, as specified in OAR 414-300-0030(5)(a);
  • dates and times of the practices of emergency procedures;
  • child abuse reports made to the Department of Human Services Child Welfare (DHS) or a law enforcement agency;
  • authorizations to administer medication to a child, as specified in OAR 414-300-0230(1)(a);
  • medications dispensed, as specified in OAR 414-300-0230(1)(d);
  • program of activities for each group of children, as specified in OAR 414-300-0295;
  • the daily schedule for each group of children, as specified in OAR 414-300-0290;
  • meals and snacks, as specified in OAR 414-300-0060(1)(d)(F);
  • water lead testing as specified in OAR 414-300-0060(1)(e);
  • related documentation and correspondence, including e-mail.
Retention and Disposal Instructions

(a) 6 years for records of water tests for lead, purge;

(b) 3 weeks for records of meals and snacks, purge;

(c) 2 years after termination of care for children’s records, purge;

(d) 2 years for all other records, purge.

Exception: In the event of a subpoena, audit, legal hold, public records or similar state or federal information request, halt until further instructed any scheduled disposal activities, including purging or transferring material to University Archives.
