Description

Document the prevention, reporting of and response to communicable diseases, including any applicable immunization and case management records (including those related to COVID-19).

These records found in all media (paper, digital, or other) may include but are not limited to:   

  • immunization records and exemption forms collected at enrollment;
  • records related to the discovery of communicable disease, including test results;   
  • case management records including names, and addresses, and other contact information of individuals who had the communicable disease (case) or were close contacts of the case; 
  • notifications;   
  • related documentation and correspondence, including e-mails, encrypted messages, etc.  

This series does not include (a) immunization information relating to a patient of the University Health Services (UHS) and submitted to or retained by UHS as part of a student's medical record and (b) employee immunization information, which are subject to UO RRS Series Medical Records and Employee Medical Information, linked to in the "Related Series" field.

Any medical interactions with a student with a communicable disease and/or their close contacts also are maintained by University Health Services in accordance with the UHS medical records retention policies. 

Retention and Disposal Instructions

5 years, purge.

Exception: In the event of a subpoena, audit, legal hold, public records or similar state or federal information request, halt until further instructed any scheduled disposal activities, including purging or transferring material to University Archives.
