Description

Document educational records for early intervention and early childhood special education services to children birth to 5 years old with developmental delays or disabilities by Early Childhood CARES. 

These records (paper, digital, or other) may include but are not limited to: 

  • Demographic information
  • Referral intake/ outcomes
  • Consent forms
  • Evaluation planning documents and other associated documents
  • Evaluation reports
  • Medical records supporting eligibility
  • Records Requests
  • Eligibility statements
  • Individualized education plans (IEP)
  • Service Calendar(s)
  • Individualized family service plans (IFSP)
  • Meeting Minutes   
  • Goal progress and review forms   
  • Behavior Support Plan (BSP)   
  • Prior Written Notices   
  • Meeting Notices   
  • Letters to Parents   
  • Copy Request Forms   
  • Medical protocols such as asthma, allergies, seizure   
  • Legal documents (Parental/Custodial designation/delegation, birth certificate, adoption, restraining orders, name changes, subpoena)

Retention and Disposal Instructions

(a) SSID (secure student identification number), child’s name, date of birth, kindergarten year, agency and county, retain in perpetuity;

(b) Educational records including records pertaining to speech language therapy, physical therapy, occupational therapy and other services: Until student reaches age 21 or 7 years after last seen, whichever is longer, purge;

(c) Supporting documentation for claims or records submitted for Oregon Health Authority, Medicaid, or other agencies, 7 years, purge;

(d) Adoption Court Orders, Physician/Medical records not used for eligibility, purge when child exits for kindergarten. 

Exception: In the event of a subpoena, audit, legal hold, public records or similar state or federal information request, halt until further instructed any scheduled disposal activities, including purging or transferring material to University Archives.
