These records found in all media (paper, digital, or other) may include but are not limited to:

• signed letter of intent;
• admissions verification reports, academic transcripts and major information, and financial aid information;
• performance reports;
• copies of required NCAA Division 1 forms: student-athlete statement, drug test consent, and HIPAA/Buckley Amendment consent/waiver form;
• release of information forms;
• personal data questionnaires;
• documentation of awards, publicity material;
• students’ requests for release from athletic programs;
• disciplinary memoranda;
• related documentation and correspondence.
