Document all investigator disclosures of financial interests.  

These records found in all media (paper, digital, or other) must include:  

• investigator disclosures of financial interests;  
• documentation of the university's review of, and response to, such disclosures (whether or not a disclosure resulted in the university's determination of a financial conflict of interest) and all actions under the university's policy or retrospective review; examples of documentation include, but are not limited to management plans and correspondence, including e-mail, that reflect major decision-making.
