Description

Document requests and authorizations related to payroll actions, including deductions. Does not include employee time sheets/time reporting records or Oregon Tax-Deferred Investment (TDI) documentation. 

These records found in all media (paper, digital, or other) may include but are not limited to:

  • new hire payroll paperwork: student employment forms (SEF) and form data; employee information form (EIF) and form data; direct deposit authorizations; disabled workers and veterans self-identification form; W-4; forms required of international employees (examples: IRS Form 8843, UO-NRA/foreign national data request form and form data, DS-19);
  • deductions authorizations, including voluntary requests (to offset union dues, insurance costs, facilities fees, such as gym memberships or parking permits fees, or charitable contributions, such as food drives) or mandatory orders (garnishments or other levies) to withhold wages from employee earnings;
  • Payroll Request Forms (PRF) and form data;
  • completed affidavits of forged endorsement; 
  • earnings request form and form data;
  • lost check forms and form data;
  • manual check requests;
  • name/gender change form and form data (payroll form);
  • overpayment notification form and form data;
  • payroll accounting adjustment forms and form data
  • payroll advance request/documentation of advances;
  • notification of preferred first name (payroll form);
  • pre-tax parking waivers;
  • request to change mailing address;
  • request for Exemption from Oregon State Withholding;
  • request for payroll check delivery through the US Postal Service;
  • request for overload compensation;
  • W2R W-2/1042-S duplicate request;
  • related documentation and correspondence, including e-mail.
Retention and Disposal Instructions

(a) 6 years after resolution for garnishments, purge;

(b) 6 years after action for all other records, purge.

Exception: In the event of a subpoena, audit, legal hold, public records or similar state or federal information request, halt until further instructed any scheduled disposal activities, including purging or transferring material to University Archives.
